Study finds wearable cardiac monitors improve diagnosis of arrhythmias

A study led by the Duke Clinical Institute found that wearable, long-term continuous cardiac monitors helped identify 52% more cases of atrial fibrillation compared with usual care, but did not lead to a reduction in hospitalizations for stroke.

The study enrolled nearly 12,000 U.S. patients aged 70 years or older with no history of atrial fibrillation. Approximately half of the patients were randomly assigned to receive a long-term (14-day) continuous monitoring device, and the other half received usual care.

The study reported that over an average follow-up period of 15 months, device recipients experienced a 52% increase in diagnosed cases of atrial fibrillation compared with those receiving usual care. There was no increase in hospitalization rates for bleeding, and no significant reduction in hospitalization rates for all strokes compared with usual care.

The study was originally designed to enroll 52,000 patients and would have been powered to determine whether screening reduces the number of strokes. A large study population is needed because strokes occur in a proportion of patients who have the following: Atrial fibrillation.

“Although the results are inconclusive, we have learned a number of lessons that can inform future research,” said Lopez, who noted that the study design, in which patients used a self-applied patch device at home and were able to enroll and be tested online in a virtual format with only remote support, could potentially be replicated in future studies.
