New CDC population data Projections from 2023 onwards show that 23 states will have more than one in three adults (35%) who are obese. Prior to 2013, no state had an adult obesity rate above 35%. Today, obesity affects at least one in five adults (20%) in every U.S. state.

“This new data highlights the need for obesity prevention and treatment options, and it starts with building healthier communities where people of all ages have safe places to exercise, and where health care and healthy eating options are accessible and affordable for everyone,” said Karen Hacker, M.D., director of CDC's National Center for Chronic Disease Prevention and Health Promotion. “Preventing obesity at a young age is important because we know that obese children often become obese adults, which is one of the reasons we are prioritizing state and community investments in effective obesity prevention.” Childcare and Family Healthy Weight Program

The 23 states with the highest adult obesity rates (over 35%) are Alabama, Alaska, Arkansas, Delaware, Georgia, Illinois, Indiana, Iowa, Kansas, Louisiana, Michigan, Mississippi, Missouri, Nebraska, New Mexico, North Dakota, Ohio, Oklahoma, South Carolina, South Dakota, Tennessee, West Virginia, and Wisconsin. Guam and Puerto Rico also had obesity rates over 35%. Adult obesity rates by state, by race, ethnicity, and location are: Behavioral Risk Factor Surveillance System (BRFSS).

Obesity impacts some groups more than others, with notable differences across race and ethnicity, as seen in the combined 2021-2023 BRFSS data.

  • Asian Adult No state has an obesity rate above 35% (out of 37 states, one territory, and DC).
  • In 16 states: White Adult Obesity rates above 35% (47 states, 2 territories, and D.C.).
  • In 30 states, American Indian or Alaska Native adult Obesity rates over 35% (out of 44 states)
  • In 34 states: Hispanic Adults Obesity rates above 35% (47 states, 3 territories, and Washington, DC).
  • In 38 states, Black Adult Obesity rates of 35% or higher (46 states, 1 territory, and D.C.).

“Obesity is a complex disease. There is a common misconception that obesity is the result of a lack of willpower or an individual's failure to eat healthily or exercise,” said Ruth Petersen, M.D., director of CDC's Division of Nutrition, Physical Activity, and Obesity. “Many factors contribute to obesity, including genes, certain medications, lack of sleep, gut microbiome, stress, access to affordable food, safe places to be active, and access to health care. Understanding these factors can help us identify potential prevention and treatment strategies.”

CDC partners with communities, states, and tribes to improve health outcomes and reduce obesity among people across the country who are at highest risk and burden of chronic disease. CDC invests in programs such as: span, hopand arrival It increases access to healthy food options, establishes safe and accessible physical activity locations, and promotes stigma-free obesity prevention and treatment programs that will fund 17 states, 50 local and tribal organizations, and 16 land-grant universities.

Obesity treatment also involves a combination of obesity medications, such as GLP-1, recently approved by the U.S. Food and Drug Administration, and health behavior and lifestyle interventions.

Obese adults have a body mass index above 30. Obesity is a disease that can lead to many health conditions, including asthma, heart disease, stroke, type 2 diabetes, some cancers, and serious outcomes from respiratory diseases. Additionally, bias and stigma surrounding a person's weight can lead to social and mental health consequences, including anxiety and poor body image.
