At least 30 children have died due to medicine shortages in part of northwest Pakistan, after the regional government closed main roads in and out of the district to try to quell a surge in deadly sectarian violence.

The Kurram district in Khyber Pakhtunkhwa province, which borders Afghanistan, has for decades been a hotbed of sectarian violence between Shiite and Sunni Muslims, and since July conflicts over agricultural land have intensified.

Violence erupted on November 21 when gunmen ambushed a convoy of vehicles and killed 42 people, most of them Shiites. No one claimed responsibility for the assault, which sparked retaliatory shooting and arson by rival groups in several areas.

At least 130 people have been killed since October. People from both groups stayed at home for fear of attack. The regional government said it would only fully open land routes to the district when armed groups on both sides surrender their heavy weapons.

Syed Mir Hassan, medical director of the main hospital in Parachinar, the district capital of Kurram, said at least 30 children had died due to the shortage of medicines.

Residents have expressed concern over a growing humanitarian crisis. We lack food, medicine, milk and fuel, said Ahbaab Ali, 25, from Parachinar. He expressed concern that people could soon run out of all basic necessities. The provincial and central governments are not paying any attention to the crisis, he said.

Kurram's population of 800,000 is roughly split between Shiites, who live to the north, on the Afghan border, and Sunnis, who live to the south.

The children's deaths sparked criticism of the provincial government, led by former Prime Minister Imran Khan's party, and the national government, led by Prime Minister Shehbaz Sharif.

Faisal Edhi, philanthropist and director of the Edhi Foundation, which provides medicine and food to people stranded in Kurram, said: People are in dire need of help. Hospitals are lacking services and supplies have been cut off. I saw hospitals that had no oxygen or medicine. X-rays and CT scans were malfunctioning. I call on the state to make Kurram its top priority and find a solution to sectarian violence.

Zahid Hussain, a security analyst and author, said the recent bloodshed was unprecedented. He said: “Two factors make the crisis so deadly: first, the rule of the Afghan Taliban in Afghanistan and the resurgence of the Pakistani Taliban in the tribal districts and their support for Sunni Muslims; and second, Iranian support for Shia Muslims in the district, many of whom belong to Iranian militias.

Various truces have been announced since the latest fighting began, as elders from both camps attempt to negotiate a lasting deal.

Analysts fear that the failure of provincial administrations to quell the violence means it could spill beyond Kurram. Hussain said: The government is trying to resolve the issue through a tribal jirga. [a council of elders]. This shows that there is no state mandate. It's worrying.
