Small research results that can detect depression through fluctuations in heart rate
A new study presented at a virtual conference at the European University of Neuropharmacology shows that depression can be detected by monitoring fluctuations in a person’s heart rate over a 24-hour period. Small-scale proof-of-concept studies certainly require more validation, but promise new biomarkers that can be easily integrated into patient monitoring systems.
Ah The body of recent research A compelling link has been found between depression and heart rate variability. Of course, so many factors affect a person’s heart rate that it was difficult to come down to this new association.
The new study aimed to investigate the association by recruiting 16 subjects with treatment-resistant major depressive disorder (MDD). The small cohort matched 16 healthy individuals, the entire group was followed for the first 4 days, and a small wearable device continuously recorded heart rate data.
“We found that people with depression had higher baseline heart rates and lower than expected heart rate variability,” said Carmen Schiweck, principal investigator of the Goethe University project. “On average, we found that patients with depression had heart rates about 10 to 15 beats per minute higher than the control group.”

Schiweck suggests that this allows researchers to observe a near-instantaneous improvement in depressive symptoms, which can be correlated with heart rate observations.
“After treatment, heart rate measurements were measured again and found that both heart rate and heart rate variability in previously depressed patients changed to be closer to what is seen in the controls.” Said Schiweck.
The final part of the study was to test a computerized classification system to see if a healthy control could identify a depressed subject based solely on heart rate data. Temporal heart rate data proved to be the most effective in identifying subjects with depression.
“Heart rate is usually high during the day and low at night,” explains Schiweck. “Interestingly, depression seems to impair the decrease in heart rate at night. This seems to be a way to identify patients at risk of developing or recurring depression.”
Using heart rate data recordings alone, the system can correctly identify 15 of 16 control subjects and 14 of 16 patients with depression. Higher resting heart rates were also correlated with depressed patients who were more likely to respond positively to ketamine treatment. This suggests that biomarkers may help classify patients with depression who would most benefit from ketamine treatment.
“Simply put, our pilot study suggests that just by measuring your heart rate for 24 hours, you can determine with 90% accuracy whether a person is currently depressed,” Schiwack said. Concludes.
The study has not yet been peer-reviewed or published in the journal. Schiweck is cautious to emphasize that the study is preliminary and requires greater validation in diverse patient populations. However, the impact of the work is compelling.
Once validated, this type of biomarker can be easily measured from many wearable devices currently available and incorporated into health monitoring apps that act as a sign of “early warning” of depression. Biomarkers can also be used to monitor the effectiveness of specific treatments and to determine the optimal treatment for a particular patient.
