Background: People who experienced the Great East Japan Earthquake (GEJE) were expected to experience additional levels of psychological burden from the stressful conditions imposed during the coronavirus 2019 (COVID-19) pandemic; Thus, suicide rates may increase. OBJECTIVE: We aimed to conduct continuous monitoring of suicide rates in the affected area after GEJE under the conditions of the COVID-19 pandemic. METHODS: This descriptive study monitored suicide rates in the coastal area of ​​Miyagi Prefecture, where disaster-related mental health activities continued in the aftermath of the severe damage caused by the tsunami. An exponential time-series analysis was performed of suicide rates into a smooth trend. Results: Although the suicide rate in the affected region was higher than the national average in February 2020, it showed a downward trend during the COVID-19 pandemic, while it showed an increasing trend in the national and unaffected regions. Limitations: Uncertainty about the immediate causes of suicide and short-term observation are the limitations of this study. Conclusion: Despite the high national suicide rate, this was not the case for the affected area. Our findings may provide important lessons for suicide prevention during the COVID-19 pandemic, which requires careful regional monitoring of suicide status and high-risk approaches such as disaster-related mental health activities.
