The WHO calls for quality care for women and newborns in the critical first weeks after childbirth
The World Health Organization (WHO) today launched its first global guidelines to support women and newborns in the postnatal period – the first six weeks after birth. This is a critical time to ensure the survival of the newborn and the mother and to support the healthy development of the baby, as well as for the overall mental and physical recovery and well-being of the mother.
Worldwide, more than 3 in 10 women and babies do not currently receive postpartum care in the first days after birth – the period when most deaths of mothers and infants occur. Meanwhile, the physical and emotional consequences of childbirth – from injuries to recurring pain and trauma – can be debilitating if left untreated, but they are often very treatable when the right care is provided at the right time.
“The need for quality maternity and newborn care does not stop when the baby is born,” He said Dr Anshu Banerjee, Director of Health and Aging for Mothers, Newborns, Children and Adolescents in the WHO. “Indeed, the birth of a baby is a life-changing moment, one that is bound by love, hope, and excitement, but it can also cause unprecedented stress and anxiety. Parents need strong health and support systems, especially women, whose needs are too often overlooked when a baby is born. ”
In addition to addressing immediate health issues, these first weeks after birth are key to building relationships and establishing behaviors that affect the long-term development and health of infants. The guidelines include recommendations for breastfeeding counseling – to help attachment and positioning after breastfeeding – and to support parents in providing responsible care for their newborns.
Over 60 recommendations to help shape a positive postnatal experience for women, babies and families.
That includes:
- High quality care in health facilities for all women and babies at least 24 hours after birth, with a minimum of three additional postnatal examinations in the first six weeks. These additional contacts should include home visits if possible so that the healthcare professional can support the transition to home care. In the case of a home birth, the first postnatal contact should take place as soon as possible, and no later than 24 hours after birth;
- Steps to identify and respond to signs of danger that require urgent medical attention in a woman or baby;
- Treatment, support and advice to help recover and solve common problems that women may experience after childbirth, such as perineal pain and breast fullness;
- Screening of all newborns for ocular abnormalities and hearing impairment, as well as vaccination at birth;
- Support to help families interact and respond to baby signals, providing them with close contact, warmth and comfort;
- Exclusive breastfeeding counseling, access to postpartum contraception and health promotion, including physical activity;
- Encouraging the involvement of partners, for example, by participating in check-ups, as well as providing support to women and caring for newborns;
- Screening for postpartum maternal depression and anxiety, with referral and management services where needed.
The recommendations detail the minimum length of hospital stay after birth and provide guidance on discharge criteria, but note that the time required will depend on individual women and babies, social context, childbirth experience, and any health issues. Additional postnatal contacts are recommended for healthy women and infants between 48 and 72 hours, between seven and 14 days, and during the sixth week after birth. If health risks are identified, more contact is likely to be needed, and treatment will be needed after the first six weeks.

These recommendations complement the trilogy of WHO guidelines for quality care for mothers during pregnancy and during and after childbirth, focused on meeting the needs of all those who give birth and their babies. They support the right to a positive experience in health care, where people are treated with dignity and respect and can actively participate in health care decisions.
