The World Health Organization (WHO) recommends a comprehensive set of smoking cessation interventions, including behavioral support provided by health care providers, digital smoking cessation interventions, and pharmacological treatments in the first smoking cessation guideline.

The guidelines are aimed at helping more than 750 million tobacco users who want to quit all forms of tobacco. The recommendations are relevant to all adults who want to quit a variety of tobacco products, including cigarettes, water pipes, smokeless tobacco products, cigars, roll-your-own tobacco and heated tobacco products (HTP).

“These guidelines mark a key milestone in our global fight against these dangerous products,” said Dr. Tedros Adhanom Ghebreyesus, WHO Director-General. “It empowers countries with essential tools to effectively support individuals to quit smoking and alleviate the global burden of tobacco-related disease.”

More than 60% of the world's 1.25 billion tobacco users – more than 750 million people – want to quit, but 70% do not have access to effective smoking cessation services. This gap exists because of the challenges facing health systems, including resource constraints.

“The enormous struggle that people face when trying to quit smoking cannot be overstated. We must deeply appreciate the strength required and the suffering endured by individuals and their loved ones to overcome this addiction,” said Dr. Rüdiger Krech, Director for Health Promotion at WHO. “These guidelines are designed to help communities and governments provide the best possible support and assistance to those on this challenging journey.”

Effective therapies for smoking cessation

Combining pharmacotherapy with behavioral interventions significantly increases the success rate of quitting. Countries are encouraged to provide these treatments for free or at reduced cost to improve access, particularly in low- and middle-income countries.

WHO recommends varenicline, nicotine replacement therapy (NRT), bupropion and cytisine as effective smoking cessation treatments.

In 2023, WHO launched a pre-qualification process for medicines for tobacco use disorders to improve global access to recommended medicines for smoking cessation. In April 2024, Kenvue nicotine gum and patch became the first NRT products pre-qualified by the WHO.

WHO recommends behavioral interventions, including brief counseling by health workers (30 seconds to 3 minutes) routinely offered in health facilities, with more intensive behavioral support (individual, group, or telephone counseling) for interested users. Additionally, digital interventions such as text messaging, smartphone apps, and online programs can be used as supplements or tools for self-management.

WHO encourages health care providers, policy makers and stakeholders to adopt and implement these guidelines to promote smoking cessation and improve the health of millions of people in need worldwide.
