With the support of the Scientific Advisory Group on the Origin of Emerging Pathogens (SAGO), the World Health Organization (WHO) published global framework help Member States to comprehensively investigate the origins of new and re-emerging pathogens. Although there are a number of tools available for investigating infectious disease outbreaks, this is the first unified, structured approach to investigating the origins of a new pathogen. This framework aims to fill that gap by providing a comprehensive set of scientific research and studies. It is the first version of a “how to” guide that will be updated as needed, based on user feedback.
As every epidemic and pandemic shows, human and animal health is threatened by the increasing risk of the emergence of known (such as Ebola, Nipah, avian influenza, Lassa and Monkeypox viruses) and new pathogens with epidemic and pandemic potential (new influenza, MERS-CoV , SARS-CoV-1, SARS-CoV-2), the ability to prevent, and when we cannot prevent, to quickly contain epidemics and identify their origins is scientifically, morally and financially more important than ever.
The WHO Global Framework lists scientific research and studies for six technical elements:
- Early research of the first identified cases/clusters/outbreaks to identify potential sources of exposure, collect samples at the source, define the characteristics of the new pathogen involved to establish diagnostic tests.
- Human research: understand the epidemiology including clinical presentation, modes of transmission, pathology and earliest presence in follow-up samples of the syndrome.
- The human/animal interface studies to identify potential animal reservoirs, intermediate hosts and reverse zoonoses.
- studies to identify insect vectors or other sources of infection as well as the earliest presence in the environment. …
- Genomics and phylogenetics studies to identify precursor strains, genomic characteristics, evolution of intermediate hosts and humans, and spatial distribution over time.
- Biosafety/biosafety studies to determine whether violations of laboratory or research activities may have been related to the first cases.
The WHO Global Framework is designed as a resource for scientists, researchers, public health authorities and researchers in Member States. It provides guidance on when and how to launch such multidisciplinary research and offers recommendations to countries on the capacities and tools needed for their successful implementation. This includes necessary capacities such as human resources, human, animal and environmental monitoring systems; biosafety and biosecurity regulations; and laboratories with testing and sequencing expertise – and the importance of sharing the findings of such research as soon as possible to guide next steps. These recommendations have been developed to align with the International Health Regulations (IHR) and use the One Health approach.
Timely and comprehensive research into the origin of pathogens is essential to prevent and contain global health crises. Findings from such research provide the basis for stopping disease outbreaks before they start, halting chains of transmission and reducing the risk of pathogens spilling over from animals to humans. They can also confirm or rule out the possibility of inadvertent breaches of laboratory biosafety and biosecurity.
To achieve this, it is critical that countries carry out these exchanges of initial findings in a rapid, complete and transparent manner as soon as the results are available to ensure the implementation of measures to mitigate further transmission and prevent further spillovers and, possibly, future pandemics.
