Positive sounds during REM sleep may help nightmares
Oct 28, 2022 – For those clinically diagnosed with “Nightmare Disorder”, learning to turn disturbing dreams into more positive ones is usually your ticket back to sleep.
However, for about a third of people, this method (called image rehearsal therapy) is ineffective.

A new study defines nightmares as “an intense negative emotional experience that typically occurs during rapid eye movement (REM) sleep.” These include aggression, interpersonal conflict, images and thoughts of failure, and emotions such as fear, anger, and sadness. Nightmare disorder is characterized by having such dreams frequently enough to cause “significant distress or impairment in social, occupational, or other important areas of functioning.”
Left untreated, the nightmarish disorder can last for decades, the authors said.
The study, conducted in Switzerland, enrolled 36 participants with nightmare disorder. All 36 attended daytime lessons in Imagery Rehearsal Therapy, which taught them to transform nightmares into positive dreams. Participants were taught to recall nightmares, turn negative storylines into more positive ones, and rehearse so-called “rewritten dreams” during the day.
Half of the participants played a special sound while practicing revisiting new positive dreams. During the nights for the next two weeks while they were asleep, sounds were played during the REM cycle.
Those who heard the sound reported significantly fewer nightmares.
“This difference showed moderate to large effect sizes and was sustainable at 3-month follow-up,” reported the authors.
They noted that both groups showed improvement because lessons for reimagining nightmares into positive dreams are known to be effective. It acknowledged that other factors may have contributed in ways beyond the control of the study design.
“Results should be replicated,” says Morgenthaler. “But I was a little excited about this new possibility.”
