CDC Issues Doxy-PEP Clinical Guidelines for Prevention of Bacterial Sexually Transmitted Diseases

Today, the CDC Clinical guidelines for patient counseling regarding doxycycline postexposure prophylaxis (or doxyPEP) for prevention of bacterial sexually transmitted infections (STIs)DoxyPEP has been proven to reduce the risk of bacterial sexually transmitted diseases (specifically syphilis, chlamydia, and gonorrhea) in gay, bisexual, other men who have sex with men, and transgender women who are at high risk for bacterial sexually transmitted diseases.
The CDC recommends that health care providers discuss doxycycline PEP with all gay, bisexual, and other men who have sex with men and transgender women who have a history of at least one bacterial sexually transmitted disease (syphilis, chlamydia, or gonorrhea) within the past 12 months. If offering doxycycline PEP, health care providers should write a prescription for self-administration of doxycycline 200 mg within 72 hours after sexual intercourse.
These guidelines were finalized after careful consideration of previous comments received from the public and experts, as well as the available scientific evidence on the subject. While we are constantly learning more about how to prevent STIs, we believe this is the right step at this time to protect the public's health. We will continue to adjust our recommendations as additional data becomes available. We know that our work does not end with the publication of these recommendations. Important questions remain, including the effectiveness of this approach for other populations, the potential for long-term development of antimicrobial resistance, and its impact on the microbiome. Going forward, we will leverage our existing data systems to monitor the uptake of doxycycline PEP and its impact on bacterial STI rates, understand doxycycline prescription rates in the country, and identify trends in antimicrobial resistance. As always, we will provide updates as more information becomes available.
Doxy-PEP is the first new STI prevention tool in decades at a time when the nation desperately needs innovation to fight STIs. To that end, we will provide guidance to funded STI programs across the country to help them determine how to most effectively use CDC resources to support implementation of Doxy-PEP.
