Mutant type 2 poliovirus isolated from sewage sample in Gaza Strip – GPEI

Variant type 2 poliovirus (VDPV) has been confirmed in Gaza. The virus was isolated from six environmental (sewage) samples taken from two different collection sites in two sub-regions within Gaza on 23 June 2024.
The mutant poliovirus strains detected in all six wastewater samples taken from two environmental monitoring sites in Khan Yunis and Deir al-Balah are genetically related to each other. The origin of the isolated viruses is currently under investigation. It is important to note that at this time the virus has only been isolated from the environment and no associated cases of paralysis have been detected.
Under the leadership of the Gaza Ministry of Health, a risk assessment is being carried out, including an assessment of surveillance sensitivity for the detection of Acute Flaccid Paralysis (AFP) and environmental monitoring, to more clearly assess immunity levels at local level.
WHO, UNICEF and partners continue to work with all local public health authorities as part of urgent efforts to mitigate the impact of the current crisis on the health of the Gaza Strip's population. The ongoing crisis continues to pose significant challenges to the capacity to implement full disease surveillance and delivery of vaccination services.
Routine immunization coverage in the Occupied Palestinian Territory (oPt) was optimal before the start of the conflict in October 2023. Polio vaccination coverage (POL3), delivered primarily through routine immunization, was estimated to be 99% in 2022. The latest report indicates that it has fallen to 89% in 2023. Currently, only 16 of the 36 hospitals are partially functional and 45 of the 105 primary health facilities are operational. The impact on the health system, insecurity, difficulty in access, population displacement, shortages of medical supplies as well as poor water quality and poor sanitation have led to reduced routine immunization coverage and increased risks of vaccine-preventable diseases such as polio.
WHO considers there to be a high risk of the variant spreading within the Gaza Strip and internationally, especially given that the current situation continues to affect public health services.
