The demands of weekdays, often dictated by school and work schedules, can lead to interrupted or deprived sleep, but new research presented at the ESC Congress 2024 suggests that people who “catch up” on lost sleep by sleeping in on the weekend could have a five-fold lower risk of heart disease.

“Sufficient compensatory sleep is associated with a reduced risk of heart disease,” said study co-author Song Yanjun of the State Key Laboratory of Infectious Diseases at Fuwai Hospital, National Cardiovascular Center, Beijing, China. “The association is even more pronounced among people who regularly experience sleep deprivation on weekdays.”

It's well known that people who suffer from sleep deprivation will “sleep in” on their days off to mitigate the effects of sleep deprivation, but there is a lack of research into whether this compensatory sleep benefits heart health.

To assess the relationship between weekend compensatory sleep and heart disease, the authors used data from 90,903 participants in the UK Biobank project, who recorded their sleep data using accelerometers and grouped them into quartiles (four roughly equal groups ranging from most to least compensated): Q1 (n=22,475) had the least compensated sleep, ranging from -16.05 to -0.26 hours (i.e. even less sleep), Q2 (n=22,901) -0.26 to +0.45 hours, Q3 (n=22,692) +0.45 to +1.28 hours, and Q4 (n=22,695) had the most compensated sleep (1.28 to 16.06 hours).

Sleep insufficiency was self-reported and defined as insufficient sleep among individuals who self-reported fewer than seven hours of sleep per night. A total of 19,816 participants (21.8%) were defined as sleep insufficient. The remainder of the cohort may have experienced occasional sleep insufficiency, but on average, their daily sleep duration did not meet the criteria for sleep insufficiency. The authors acknowledge this is a limitation of the data.

Hospital admission records and cause of death registry information were used to diagnose various cardiac diseases, including ischemic heart disease (IHD), heart failure (HF), atrial fibrillation (AF), and stroke.

Over a median follow-up period of almost 14 years, participants in the most compensatory sleep group (quartile 4) were 19% less likely to develop heart disease than those with the least compensatory sleep (quartile 1). In the subgroup of patients who were sleep deprived every day, those with the most compensatory sleep had a 20% lower risk of developing heart disease than those with the least compensatory sleep. The analysis found no differences between men and women.

Co-author Liu Zezhen, from the State Key Laboratory of Infectious Diseases at the National Cardiovascular Center at Fuwai Hospital in Beijing, China, added: “Our results show that for a significant proportion of the population in modern society who suffer from sleep deprivation, people who get the most 'make-up' sleep on weekends have a significantly lower incidence of heart disease than those who get the least.”
