Working in laboratory mouse models of stroke and stroke patients, UCLA researchers have identified loss of brain connections where stroke is far from the site of stroke injury. Brain cells far from the site of stroke are cleaved from other neurons. As a result, the brain network does not fire together for movement, walking, etc.

The UCLA team discovered that some of the connections lost after a stroke occur in cells called palmi neurons. This type of neuron helps to produce a brain rhythm called gamma oscillations. It is called gamma vibration. This allows neurons to join together to form a coordinated network to generate actions such as movement. A stroke causes the brain to lose gamma vibrations. Successful physical rehabilitation in both laboratory mice and humans restored gamma oscillations to the brain, and in a mouse model, repaired the lost connections of palmi neurons.

Carmichael and his team then identified two candidate drugs that could produce gamma oscillations after a stroke. These drugs work specifically to excite palmi neurons.

Researchers found that DDL-920, one of the drugs developed at Dr. Varghese John, who co-authored the study, led to a significant recovery in motor control in mice.

This study has two major areas of impact. First, we identify the brain substrates and circuits underlying the effects of brain rehabilitation. Second, this paper identifies unique drug targets in the brain circulation of this rehabilitation, and promotes recovery by mimicking the main effects of physical rehabilitation.

Further research is needed before examining the safety and efficacy of DDL-920 in human trials.
