After almost choking when a patient accidentally inhaled a lung medicine capsule through the wrong part of the inhaler Jazz, he warned of a potential danger to the concerned pharmacist, Medicine and Healthcare Products Regulatory Agency (MHRA).
Jazz, pharmacist and Yellow Card reporter said:
If this happened to one person, I thought it could happen to others, and I wanted to keep the patient safe, so I reported it on a yellow card.
MHRA conducted a detailed investigation and found potential confusion when using drugs and devices together. Then we worked with the manufacturer to improve the safety information.
The product now comes with clear instructions on how to properly insert the capsule into the device, including a warning about choking and an easy-to-understand picture.
MHRA has also warned doctors, pharmacists and healthcare professionals across the UK about safety risks.
It shows that all information is important and can make a big difference in patient safety.
Mick Foy, manager of the MHRAs Vigilance and Risk Management of Medicines Division, said:
Patient safety is at the heart of everything we do and everyone plays a role in the safe use of medicines.
MHRA uses Yellow Card reports to reinforce investigations and advice on how to safely use products to protect patients. Whether you have experienced side effects, defective medical devices, or witnessed potential problems, your report on the yellow card plan will help us take steps to protect everyone.
Throughout World Patient Safety Day week, MHRA is reminding the public to report safety issues related to pharmaceuticals and medical products using the Yellow Card Scheme, supporting this year’s World Patient Safety Day focus on healthcare worker safety. MHRA commends the vital role of healthcare professionals in reporting and reporting patient safety issues, including reporting via yellow card.
Anyone can use the yellow card scheme to report side effects of suspected medicines, accidents involving medical devices, and defective or fake medical products.
Reports can be made via the Yellow Card website, the mobile app on the Google Play Store or the Apple App Store, toll-free (0800 731 6789, 9am-5pm Monday-Friday), or by reporting a problem to a medical team who can report Can be submitted. Reports on their behalf.
How a pharmacist made a difference in patient safety
All reports are important and help MHRA investigate potential issues and monitor the safety and quality of medical products in the UK.
If you suspect a safety issue or accident related to a medical product on the Yellow Card website, learn more or report it.
Note to editors
The Drug and Medical Products Regulatory Authority is responsible for regulating all pharmaceuticals and medical devices in the UK by ensuring they are functional and safe to an acceptable level. All of our work is backed by strong, fact-based judgments so that the benefits justify all risks.
MHRA is the center of pharmaceutical and medical product regulatory agencies, including the National Institute for Biological Standards and Control (NIBSC) and Clinical Practice Research Datalink (CPRD). MHRA is the executive body of the Ministry of Health and Social Services.
The Yellow Card scheme is a UK system for collecting and monitoring information about suspicious concerns or incidents related to medicines and medical devices. The plan is run by MHRA and currently relies on voluntary reporting of suspected problems from health care professionals and patients. The purpose of this scheme is to provide an early warning that the safety of a product may require further investigation.
For a drug to be considered safe, the expected benefits must outweigh the associated risk of adverse reactions. Any medication can cause a reaction. However, most people take the medicine without serious side effects. Medical professionals should be able to discuss this information with patients, parents and caregivers, take care to detect any suspected safety issues, and report immediately through a yellow card plan.
