After almost choking when a patient accidentally inhaled a lung medicine capsule through the wrong part of the inhaler Jazz, he warned of a potential danger to the concerned pharmacist, Medicine and Healthcare Products Regulatory Agency (MHRA).

Throughout World Patient Safety Day week, MHRA is reminding the public to report safety issues related to pharmaceuticals and medical products using the Yellow Card Scheme, supporting this year’s World Patient Safety Day focus on healthcare worker safety. MHRA commends the vital role of healthcare professionals in reporting and reporting patient safety issues, including reporting via yellow card.

Anyone can use the yellow card scheme to report side effects of suspected medicines, accidents involving medical devices, and defective or fake medical products.
