Rapid test for SARS-CoV-2 antigen detection in delta mutants

Given the emergence of novel SARS-CoV-2 mutants of concern, it is necessary to investigate the diagnostic performance available for these new mutants. Previously, nine commercially available Ag-RDT analytical sensitivity tests were performed on the first three identified mutant strains of concern (alpha, beta, and gamma) and one previously of interest mutant strain (zeta). A virus-free Ag-RDT buffer was used as a negative control. The results were read independently by two individuals. A visible test band in the presence of a visible control band was considered a positive result.The performance of the test to detect the delta variant was similar to most other variants of Ag-RDT. A single test, the SureStatus COVID-19 Antigen Card Test (Premier Medical Corporation), showed higher sensitivity to alpha, beta, and gamma variants compared to delta variants. Conversely, the Flowflex SARS-CoV-2 Antigen Rapid Test (ACON Laboratories) was more sensitive to deltas than other Ag-RDT kits ("Appendix pp1–2). Compared to B.1.610, delta variants such as alpha, beta, and gamma variants showed higher sensitivity.In this study, the accuracy of 11 Ag-RDTs for detecting the variant of concern was determined. Analytical validation with cultured viruses may be a substitute for clinical accuracy, but not a substitute for clinical evaluation.
