In the United States to end vaccine funds for poor countries

The Trump administration intends to end the financial support of the United States in Gavi, the organization that helped buy critical vaccines for children in developing countries, saving millions of lives during the last quarter of a century, and considerably reducing support for efforts to combat malaria, one of the largest killers worldwide.
The administration has decided to continue certain key subsidies for drugs to treat HIV and tuberculosis, as well as food aid to countries faced with civil wars and natural disasters.
These decisions are included in a 281-page spreadsheet that the United States agency for international development sent to the congress on Monday evening, listing the foreign aid projects that it plans to continue and terminate. The New York Times obtained a copy of the spreadsheet and other documents describing the plans.
Documents provide a radical overview of the extraordinary scale of administrations withdraw from an effort of half a century to present the United States in developing as a compassionate ally and to lead the fight against infectious diseases that kill millions of people per year.
The cover letter details the skeletal remains of the USAID after the cuts, most of its funding is eliminated, and only 869 of more than 6,000 employees are still in active service.
In all, the administration decided to continue 898 USAID prices and end 5,341, the letter said. He indicates that the remaining programs are worth $ 78 billion. But only $ 8.3 billion is not obliged funds of funds still available to get rid of. Since this amount covers prices that take place for several years in the future, the figure suggests a massive reduction in the $ 40 billion that USAID has used each year.
A spokesperson for the State Department, who now manages what remains of the USAID, confirmed that the licensees on the list were correct and declared that each allocation was individually revised to align with the priorities of agencies and the administration, and the licensees were executed when Secretary Rubio determined that the reward was incompatible with national priorities of interest or national agency.
The Memo at the Congress presents the foreign assistance plan as a unilateral decision. However, because individual health programs expenses such as HIV or vaccination are allocated to Congress, it is not clear that the administration has the legal power to end these programs. This question is currently being advocked in several judicial disputes.
Among the licensed programs, there is funding from the United Nations Food and Agriculture Organization, which leads to surveillance of diseases that can be transmitted from animals to humans, including the flu is in 49 countries. Certain major programs to monitor and fight malaria, one of the best child killers in the world, have also ended.
Dr. Austin Demby, Sierra Leone Minister of Health, who relies on Gavis' support to help buy vaccines, said he was shocked and disrupted by the decision to end US funding and warned that ramifications would be felt around the world.
It is not only a bureaucratic decision, there is a life for children at stake, world health security will be at stake, he said. Supporting Gavi in Sierra Leone is not only a problem with Sierra Leone is something that the region, the world, benefits.
In addition to trying to reach all children with routine vaccinations, Sierra Leone is currently fighting against an MPOX epidemic, for which Gavi has provided both vaccines and critical support to deliver them, he said.
We hope that the US government will continue to be the world leader he has always put money in Gavi is not an expense, it is an investment, said Dr. Demby
Gavi would have saved the lives of 19 million children since its creation 25 years ago. The United States contributes 13% of its budget.
The grant licensed in Gavi was worth 2.6 billion dollars until 2030. Gavi was on a commitment made last year by President Joseph R. Biden Jr. for his next financing cycle.
New vaccines that promise to save millions of lives in low -income countries, such as that to protect children from serious malaria and another to protect adolescent girls from virus that causes cervix cancer, have recently become available, and Gavi expanded the support portfolio that could be given to these countries.
The loss of American funds will bring the ability of organizations to continue to provide its range of basic services such as immunization for measles and polio to an increasing population of children in the poorest countries, not to mention the development to include new vaccines.
According to Gavis, its own estimate, the loss of American support could mean that 75 million children do not receive routine vaccinations over the next five years, with more than 1.2 million children who die accordingly.
The United States has been among the best donors in the organization since its creation and have become the most important in the COVVI-19 pandemic. While European countries have historically provided significant funding, many are now reducing foreign aid spending because they are confronted with the change in American policy in Ukraine and the United States demands that it increase its defense expenses. Japan, another adult from Gavi, has trouble deprecating a currency.
DRE Sania Nishtar, Managing Director of Gavis, said that she hoped that the Trump administration would reconsider the decision to end her. Gavis' work keeps people everywhere, including Americans, in complete safety, she said. In addition to protecting individual children, vaccination reduces the possibility of large epidemics. The organization maintains global stocks for diseases against diseases such as Ebola and cholera, deployment in rapid response efforts for epidemics.
The Gavis structure forces countries to pay part of the cost of vaccines, their share increasing as income levels increase; Middle income countries are weaned for support.
Although the administration has repeatedly declared publicly that its foreign aid examination process had been concluded, the information in the documents suggest that there is still a certain fluidity in which the programs will survive. The staff of a major malaria program who were dismissed weeks ago, and who appears on the list of canceled projects sent to the Congress, for example, was informed on Monday that it was restored.
Nevertheless, the response cuts to malaria are deep. Although prices finance the purchase in bulk of malaria beds and treatments have been preserved, many programs to deliver these efforts and other efforts to combat malaria in individual countries such as Cameroon and the most affected Tanzania in the world have been terminated. Some organizations with prices that have not been officially canceled have not received any funds for more than two months and have withdrawn. Without them, there is no one to take port treatments at local clinics or deliver them to children.
The service note indicates that 869 USAID staff worked last Friday, while 3,848 were on administrative leave and 1,602 are being dismissed. Out of 300 probations employees who were initially dismissed, 270 returned to work following an order of the court prohibiting their dismissal.
