Remethinostat, an HDAC inhibitor associated with positive clinical outcomes in patients with basal cell carcinoma

Remethinostat showed signs of clinical efficacy in patients with basal cell carcinoma (BCC), according to a study published in Clinical cancer research.. Rementinostat, a topical cream, is the first class inhibitor of histone deacetylation.

BCC is the most common form of skin cancer and is usually treated with surgical resection. However, studies have shown that resection can be a costly and costly treatment, especially for patients with multiple BCC lesions. Topical creams can act as an alternative strategy, but only superficial subtypes of BCC are treated with currently existing topical therapies.

Researchers have previously identified histone deacetylase (HDAC) inhibition as a promising therapeutic approach to BCC, and remethinostat is an HDAC inhibitor designed to be ineffective when absorbed beyond the skin. .. Thus, drug activity is localized to skin lesions, as opposed to systemic HDAC inhibitors associated with various toxicities.

To test Remethinostat for the treatment of BCC, researchers enrolled 30 patients with at least one BCC with a diameter of 5 mm or more at diagnosis. Participants applied Remethinostat gel to the tumor three times daily for 6 weeks.

Eight weeks later, the remaining tumor was surgically resected and histologically examined. 90% of participants were identified as non-Hispanic Caucasians, and nearly 50% reported a history of skin cancer. The majority of tumors have nodular or superficial histology, and these tumors were found in both sun-exposed and unexposed parts of the body.

The final analysis included 33 tumors, of which 69.7% responded to topical treatment, including 17 complete and 6 partial responses. Tumor diameter decreased by an average of 62.3% and tumor area decreased by 71.5%.

It was observed that multiple BCC subtypes responded to remethinostat. Six superficial BCC tumors had a 100% response rate, with five complete and one partial responses. The response rate for nodular BCC was 68.2% and that for infiltrating BCC was 66.7%. No response was observed in the two micronodular tumors. Future studies will analyze the lifetime of these responses.
