- Set up an account Scan your driver’s license or ID with your smartphone’s camera. Create a username using the registration code.
- After logging in, press the “my Vaccine Record” button on the main wallet page.

- Please check your name, name, date of birth, and gender.
- Please enter your phone number or email address.
- Click “Send”.
- The app says it can take up to 10 seconds for the record to load.
- The COVID-19 Vaccination Record Card will be displayed and can be downloaded to your device.
