Association between hormone replacement therapy and increased risk of dementia in later life
British Medical Journal (BMJ): Menopausal hormone therapy and dementia: a national nested case-control study
A large Danish observational study reported that menopausal women on hormone replacement therapy (HRT) had an increased risk of developing dementia later in life.
The study used data from health registries, including detailed histories of hormone therapy and diagnoses of dementia cases in memory clinics.
This result suggested that both short-term and long-term use of HRT may be effective. The study took into account certain factors that may influence the risk of dementia, such as low socioeconomic background, but other risk factors such as smoking and social isolation could not be considered.
Dr Susan Koolhaas, Executive Director of Research and Partnerships, Alzheimer’s Research UK, said:
“Dementia is the number one killer of women in the UK. Women are almost twice as likely as men to be affected, and this difference cannot be explained by differences in life expectancy alone. It is important to understand what contributes to risk, as it may help develop prevention strategies for those most at risk in the future.
Understanding whether hormone replacement therapy (HRT) has a role in dementia risk has become a priority of the research community in recent years. However, such studies have so far been inconclusive and contradictory, with some suggesting that HRT reduces the risk of dementia while others suggest that it increases it. Reasons for this may include different ages at which HRT was performed, different ways in which HRT was used, and different types of HRT he used over the years.
This large Danish national study found an association between short- and long-term HRT use and an increased risk of dementia. Unfortunately, this study did not take into account some of the other factors known to be associated, such as social isolation, smoking, and dietary factors such as alcohol. We don’t have a definitive answer as to why.
Women need to understand the implications of the benefits and risks of deciding to take HRT, and for now, as far as dementia is concerned, more research needs to await more definitive answers.
