Updated guidelines rethink care for people with hemorrhagic stroke
According to the latest guidelines for intracerebral hemorrhage (also known as hemorrhagic stroke), many are widely considered standard care for people suffering from this less common but dangerous type of stroke. No skill required.
The revised guidelines of the American Heart Association / American Stroke Association, published in the journal Stroke on Tuesday, include recommendations covering all aspects of care, from surgery to compression socks. The last revised guidelines in 2015 also focus on supporting and training home caregivers.
“We believe that the broad knowledge presented in the new guidelines will lead to significant improvements,” said Dr. Stephen M. Greenberg, chair of the group that wrote the update.
Intracerebral hemorrhage accounts for about 10% of the approximately 800,000 strokes that occur each year in the United States. It is one of the most deadly types of stroke, with a mortality rate of 30% to 40%.
Greenberg, a professor of neurology at Harvard Medical School, said: Vice-Chairman of the Department of Neurology, Massachusetts General Hospital in Boston.
Primary intracerebral hemorrhage is different from cerebral hemorrhage caused by external factors such as trauma. Typical causes include uncontrolled high blood pressure and age-related degeneration of cerebrovascular disease. This condition affects blacks and Hispanics at a disproportionately high rate.
Such strokes are expected to continue to be a serious health concern as the population ages, as the likelihood increases rapidly with age.
The new guidelines cite studies that show that:
– Minimally invasive surgical procedures may help treat some hemorrhagic strokes.
– Antiepileptic and antidepressant drugs do not appear to contribute to overall health unless the patient already has a seizure or depression. Antiepileptic drugs did not contribute to improved functionality or long-term seizure control, and antidepressants increased the likelihood of fractures.
– Administering steroids to prevent complications from hemorrhagic stroke is ineffective.
– Platelet transfusions can exacerbate the condition of stroke survivors unless used during emergency surgery.
– Wearing compression socks or stockings is not effective to prevent dangerous blood clots after a hemorrhagic stroke. If you started the same day you were diagnosed with a hemorrhagic stroke, it may help to wrap your lower legs and feet in inflatable boots.
“This is an area where we still have a lot to do,” Greenberg said. “It is unclear whether even specialized compression devices reduce the risk of deep vein thrombosis or improve the overall health of people with cerebral hemorrhage, especially within the first 24-48 years. Further research is needed on how to help. Time for first symptoms. “
People with stroke bleeding have increased pressure in the brain after the bleeding, which can damage brain tissue. These people should be considered candidates for immediate surgical procedures to relieve pressure.
“Currently, there is considerable evidence that minimally invasive surgery may improve a patient’s chances of survival after moderate or massive intracerebral hemorrhage,” Greenberg said. It is not very clear what kind of surgical procedure will improve the chances of survival and recovery, which is “our ultimate goal”.
Anticoagulants can increase complications and death, so this update provides advice on canceling immediate administration of these drugs.
The guidelines also recommend education, hands-on support, and training for the family to find out what they can expect during rehabilitation.
Educating caregivers has been shown to improve the level of activity and quality of life of stroke survivors. Practical support and training, such as how to walk safely with the patient, can help you practice rehabilitation at home and may improve the patient’s standing balance.
“People need additional help with these lifestyle changes, even if they move around, curb alcohol use, or eat healthier foods,” Greenberg said. Mr. says. “This all happens after they are discharged. We need to make sure that the family is provided with the information they need to help them properly.”
The guidelines emphasize the importance of having an interdisciplinary team plan for recovery.
The authors also recommend the development of a community health system that provides immediate treatment for hemorrhagic stroke and allows rapid transfer of people to facilities equipped with neurocritical treatment and neurosurgery units.
The guidelines outline several areas for future research, such as how quickly people can get back to work, drive, and participate in other social engagements. According to the guidelines, health care professionals also need more information on safe-level exercise and sexual activity recommendations.
