Lake County residents eligible for Phase 1B can now schedule COVID-19 vaccination appointments
Lake County, Ohio — Lake County General Health District has launched a schedule list for all under Phase 1B of the COVID-19 Vaccination Program.
Anyone who is a Phase 1B qualified Lake County resident (aged 65 or older or has a severe congenital, developmental, or early-onset medical disability) can sign up.
Residents who fill out the form will be contacted at a later date with instructions to schedule a COVID-19 vaccination appointment.
Residents working in schools from kindergarten to high school returning to face-to-face study on March 1st will be vaccinated in collaboration with the school district and should not fill out this form.
The COVID-19 vaccine is very limited and the number of vaccines arriving in the county is unknown, so filling out the form does not guarantee vaccine reservations.
