One covid survivor in each person still has symptoms two years after infection, researchers say
today BMC MedicineCatalonia, Spain, researchers estimate that 23% of adult Covid-19 survivors develop long covid, with 56% of those (8 (13%) of infected people) experiencing symptoms in two years.
The team evaluated 2,764 Covid-19 survivors from a population-based cohort established prior to the pandemic, and followed up on SARS-COV-2 and immunoglobulin (IG)G antibodies concentrations against clinical, vaccination, sociodemographic, and lifestyle factors in 2020, 2021, and 2023. Symptoms stem from patient questionnaires and electronic health records, and participants were given blood samples at study visits.
647 long-term patients were compared with 2,117 infection controls without long-term symptoms.
“There are multiple long-term mechanisms and are still unknown,” the investigator wrote. “Identifying subtypes and disease courses over time can help to unravel these mechanisms.”
Three subtypes have been identified
Between 2021 and 2023, 23% of participants developed long-term symptoms, and 56% of those infected in 2021 reported symptoms for two years. The researchers identified three long covid subtypes: mild neuromuscular (51.6%), mild respiration (20.6%), and severe multiorgan (27.8%). [RR]1.61).
The most common long-term symptoms were neurological (63%), muscle (39%), respiratory (28%), psychological and psychiatry (21%). Women were more likely to report neurological and muscle symptoms than men, and respiratory symptoms were more common in men.
Long-term patients who were first infected during the omicron period had fewer long-term symptoms, similar to those previously infected, but researchers said it could be caused by milder infectious diseases or general immunity to the virus.
Risk factors include female gender, age under 50 years old, low socioeconomic status, severe covid-19 infection, high vaccinated IgG levels, obesity, chronic disease, especially asthma or chronic obstructive pulmonary disease (COPD) and depression/anxiety. People with mild or moderate COVID-19 were more than three times more likely to have prolonged symptoms as people without symptoms (RR, 3.10), and those with severe infections were nearly 10 times more likely (RR, 9.88).
Vaccination protection
Protective factors were pre-infection or within 3 months of infection, with high levels of omicron variant infection, physical activity, and sleep 6-8 hours per night. “The finding that pre-infection vaccinations still provide long-term protective effects, even when the severity of Covid-19 suggests improved infection control and modulation of immune responses by vaccines,” the study authors wrote.
