How will OPTIMA’s findings change practice for safely omitting chemotherapy using Prosigna (PAM50) in high-clinical-risk node-positive ER+HER2 early-stage breast cancer?
Dr. Sivapiragasam: The OPTIMA exam is one of the most important exams that we look forward to hearing more about here at ASCO. This study examines ways to individualize treatment for patients with ER-positive early-stage breast cancer in the adjuvant setting.
We understand that not all hormone-positive breast cancer patients will benefit from chemotherapy. It may be effective for some cancers that have a high proliferative rate, but as we know, some luminal cancers that are low proliferative, highly ER sensitive, and are very affected by the ER do not benefit from chemotherapy. And this study attempts to address that question using an assay called PAM50.
Now, we’re already using Oncotype DX assays, MammaPrint, and a lot of other genomic elements that are really helping us make that decision. And TAILORx and RxPONDER are two large studies that use oncotypes as a way to predict oncotypes in patients without negative disease or positive disease. However, these studies had some limitations, as RxPONDER included patients with up to three positive lymph nodes. Therefore, we do not have data to examine personalization for more than three nodes.
Therefore, the OPTIMA trial is aimed at that population, specifically patients with up to nine positive lymph nodes. Alternatively, if there was node-negative disease, the tumor must be 3 cm or larger. Approximately over 4,400 patients were enrolled, with two distinct groups. The first group is collecting patients who are receiving standard treatment, all receiving chemotherapy. And in the laboratory department, patients undergo PAM50 assays. Based on your test results, you will be randomly assigned to receive either chemotherapy and endocrine therapy or endocrine therapy alone. And the cutoff they’re using is 60.
For premenopausal women, the most important thing is forced suppression of ovarian function through endocrine therapy. This is one of the gold standards you would expect to see in a non-positive population. So in this study, if you look at the primary results, it’s actually a non-inferiority study trying to prove that using the ROR score to select chemotherapy decisions is non-inferior to standard chemotherapy across all approaches. This study is positive in that regard, showing that the approach of using ROR to make these decisions is noninferior to standard chemotherapy.
Given the small absolute difference in 5-year IBCFS (91.5% vs. 90.4%), how do you interpret that the non-inferiority margin is met?
Dr. Sivapiragasam: it’s true. So the margin they were expecting was less than 3, and the absolute number they got seems to be 1.6. Therefore, this study definitely meets the expectations of their goals and is therefore considered a prospective study.
