The researchers conducted a prospective longitudinal cohort study to assess monthly patterns of anxiety and depression during pregnancy. Pregnant women were included if they had at least one previous episode of major depressive disorder and were treated with sertraline, fluoxetine, citalopram, or escitalopram instead of the current episode. Women with potential bipolar disorder or prenatal depression were not included.

Participants completed the assessment once a month from the start of the study (less than 18 weeks gestation) to delivery, and 6 and 14 weeks after delivery. Researchers used a set of screening tools, including a 10-item Edinburgh Postnatal Depression Scale and a 7-item Generalized Anxiety Disorder Scale, to assess the severity of pregnancy and postpartum depression and anxiety symptoms. Women were grouped into “trajectories” (or patterns) of depression and anxiety based on the severity of their symptoms. Overall, 88 women enrolled in the study and 77 provided complete perinatal and postnatal data. All study participants were identified as female, not transgender, non-binary, or otherwise. Women averaged 34 years and had an average gestational age of about 13 weeks. About 90% of the participants were Caucasian.

The majority of patients were also diagnosed with at least one psychiatric comorbidity. Almost 80% had a lifelong anxiety disorder, 10% had a substance abuse disorder during the last year, and 16% had a lifelong eating disorder.

About 18% of women who took SSRIs during pregnancy showed minimal depressive symptoms, 50% were mild, and 32% showed subthreshold or clinically related depressive symptoms. Regarding anxiety, 7% of women were asymptomatic, 53% were minimal, 18% were increased, and 23% were mild.
