In 2020, just two weeks into Pride Month and on the anniversary of the deadly Pulse shooting at an Orlando LGBTQ club, and in the midst of a pandemic, the Trump administration issued new regulations that: In 2019, Trump proposed cutting more than $1.35 billion, or 29%, from PEPFAR (the President’s Emergency Plan for AIDS Relief), the largest cuts ever proposed to global HIV programs. PEPFAR is the U.S. government’s overseas AIDS program. In 2017, Trump proposed cutting PEPFAR by 17%, as well as hundreds of millions of dollars in funding from the Global Fund to Fight AIDS, Tuberculosis and Malaria; and $627 billion from Medicaid, the insurance that about 40% of the estimated 1.1 million people living with HIV in the United States rely on for care. In 2018, the Trump administration announced a new office within the Department of Health and Human Services to protect religious health professionals who seek to discriminate against LGBTQ patients and others. Trump has a documented history of spreading misinformation about HIV/AIDS dating back to 1993, such as suggesting that the virus could be spread through kissing; and that people who test positive often deliberately go on a rampage to intentionally spread the virus. Despite research and reporting showing that LGBTQ people have suffered disproportionately negative health impacts from COVID-19, Trump has routinely downplayed the severity of the coronavirus pandemic and often contradicted public health experts and members of his own administration in their direst warnings about the virus throughout his presidency, according to an NPR investigative report. Trump’s newly chosen running mate, J.D. Vance, is staunchly opposed to lifesaving health care for transgender Americans. In July 2023, Vance introduced federal legislation to criminalize providing health care to transgender youth, including hormones or surgery, punishable by up to 25 years in prison. Under no circumstances should doctors be allowed to perform these horrific and irreversible surgeries on minors, Vance said in a statement at the time. His version of the bill also sought to block taxpayer funding for such care, including coverage under the Affordable Care Act; prohibit universities from offering courses on transgender health care; and deem noncitizens who supported such procedures ineligible for visas or admission to the United States. Vance’s proposed bill contradicts the recommendation of every major medical association and health authority in the world, all of which support health care for transgender people and youth and have increasingly decried the misinformation spread by those who oppose the case. More than 30 statements here. Vance opposed the Respect for Marriage Act, which enshrined federal protections for same-sex and interracial marriages, calling the protections a bizarre distraction. In June 2024, Vance voted against a bill that would have guaranteed access to in vitro fertilization (IVF).

He appointed three anti-abortion justices to the U.S. Supreme Court, creating a majority to overturn Roe v. Wade, endangering patients, families, and the safety of medical care across the country. He bragged about being responsible for overturning Roe: “If it wasn’t for me, with Roe v. Wade, you wouldn’t even be talking about this stuff. For 54 years, they tried to repeal Roe v. Wade, and I did it and I’m proud to have done it,” Trump said. He recommitted to criminalizing abortion, telling Time that he would not interfere in decisions about whether to monitor or prosecute pregnant women. He declined to say whether his second administration would enforce the Comstock Act, a zombie law from 1873 that banned the mailing of contraceptives and abortion-inducing drugs. Trump claims that his current position on abortion is that now that we have abortion where everyone wanted it legally, the states will determine that through voting or legislation, or maybe both. In an official campaign video, Trump vowed on his first day to revoke existing policies supporting health care for transgender people and youth, calling such care chemical, physical, and emotional mutilation. He pledged to sign a new executive order directing every federal agency to cease all programs that promote the concept of sex and gender transition at any age. Trump has promised to ask Congress to permanently ban the use of federal funds to promote or pay for health care for transgender Americans, without distinguishing between care for adults or minors. Trump’s current proposals would end Medicare and Medicaid funding for hospitals and health care providers that provide health care to transgender youth. Under Trump’s direction, the Justice Department and the Education Department will investigate hospitals, health care providers, and/or schools and teachers that attempt to support transgender people. Trump has said he will ask Congress to pass a bill establishing that the only genders recognized by the United States government are male and female and that they are assigned at birth, which would erase transgender and non-binary people from legal existence. The Republican Party’s 2024 platform refers to radical gender ideology, gender indoctrination, and gender madness when it refers to the intention to defund programs that support transgender health care.

81% of likely 2024 voters, 83% of undecided voters, and 73% of Trump voters believe health care decisions for transgender youth should be made by parents. A majority of all registered and potential 2024 voters say they would oppose a political candidate [who] frequently discusses restricting health care and sports participation for transgender youth. All categories of voters—LGBTQ, registered voters, Trump voters, and swing voters who say they will vote for either party—overwhelmingly agree that Republicans should stop focusing on restricting women’s rights and banning health care for transgender youth and instead focus on addressing inflation, creating jobs, and health care costs. 94% of LGBTQ voters, 76% of registered voters, 76% of likely 2024 voters, and 82% of swing voters agree. 72% of LGBTQ survey respondents reported negative impacts on their mental health and emotional well-being caused by the current state of political discourse in our country.
According to the 2022 U.S. Transgender Survey, transgender Americans have long faced discrimination in health care, from insurance issues to verbal harassment, denial of treatment, to interacting with a health care provider who doesn’t know how to properly treat transgender people. Survey participants also reported being denied routine health screenings and sexual health exams. The survey also found that transgender people are more likely than the general U.S. population to be uninsured.
