PHS290 may help predict the lifetime risk of metastatic or fatal prostate cancer

PHS290 may help predict the lifetime risk of metastatic or fatal prostate cancer

A new screening algorithm based on a polygene hazard score that integrates 290 genetic variants (PHS290) can accurately stratify a diverse population of patients for their lifetime risk of developing metastatic or fatal prostate cancer. I was able to do it.1

The PHS290 scoring algorithm reveals that the genetic risk of prostate cancer differs between men of different ancestry. In particular, men of African descent were estimated to have the highest risk of developing metastatic prostate cancer and dying from this disease compared to men of other ancestry.

The results were presented at the ASCO Genitourinary Cancer Symposium 2022 by Meghana Pagadala of the VA Health Care System in La Jolla, California.

“Comparison [with] In other risk tests, the scoring algorithm uses nearly 300 variants linked to inherited genetic information, but some commercial tests currently available are: [the] Expression of only 12 cancer-related genes and a handful of reference genes, “Pagadara said in a news release.2

Pagadala commented that previous genetic risk models were developed primarily from the background of European ancestry and may exacerbate existing health inequalities in prostate cancer incidence and outcomes.

The research team sought to determine if PHS290 was associated with a risk of metastatic or fatal prostate cancer in racially and ethnically diverse populations.

The effectiveness of PHS290 was assessed in a large multi-ancestral cohort of 582,515 participants in the VA Healthcare System Million Veteran Program.

Participants were grouped into European, African, Hispanic, or Asian ancestors. The polygene hazard score was derived from the participant’s genotypic dose for 290 single nucleotide polymorphisms. The three clinical endpoints of this study were the age at which prostate cancer was diagnosed, the age at which metastatic prostate cancer was diagnosed, and the age at which prostate cancer died.

The results showed a strong association between the PHS290 risk score and age of death from prostate cancer, metastatic prostate cancer, and any prostate cancer in the entire data set and in each of the four ancestral groups ().P <10-16).

Men with the top 20% of PHS290 scores had a 4.41 (95% CI, 3.9-5.02) higher risk of death compared to men with a genetic risk score of at least 20%.

In addition, men in the top 20% of PHS290 scores were 5.7 times more likely to develop prostate cancer and 4.2 times more likely to develop metastatic prostate cancer than men in the bottom 20% ().P <10-16).

In the multivariate model, the corresponding hazard ratios for prostate cancer mortality were 4.26 (95% CI, 3.66-4.9) for European ancestors and 2.4 (95% CI, 1.77-3.23), 4.72 (95) for African ancestors. It was% CI, 2.68). -8.87) For Hispanics 10.46 (95% CI, 2.01-101.0) For Asians.

Even when describing family history and ancestral groups, PHS290 remained a strong independent predictor of prostate cancer mortality.

On average, PHS290 risk scores were high among African-American men and were associated with metastases. Men of African ancestry were 1.8 times more likely to develop prostate cancer, 2.3 times more likely to develop metastatic prostate cancer, and 1.9 times more likely to die of prostate cancer than other ancestors (P <10-16).

In conclusion, the research team writes: “PHS290 stratified US veterans of diverse ancestry for the lifetime risk of metastatic or fatal prostate cancer. Predicting the genetic risk of fatal prostate cancer with PHS290 is May help individual decisions regarding prostate cancer screening. “
