Mammography Controversy: Early Screening Saves Lives
When should women start having mammograms? This is a simple question that should give you a clear answer. But conflicting screening guidelines over the past few years have left women at a loss for life-saving care. Fortunately, that may change, thanks to new guidance from the United States Task Force on Preventive Services (UPSTF).
Until recently, the UPSTF recommended that women at average breast cancer risk begin biennial mammograms at age 50. In early May, the UPSTF reversed policy and issued a draft recommendation that women begin screening at age 40 and continue to have mammograms every other year until that age. 75. Major medical associations, such as the American Cancer Society (ACS), still recommend that average-risk women over age 40 have annual rather than biennial mammograms, but a shift to early initiation of screening with the UPSTF. is a step in the right direction.
First, the move will lead to a reduction in mortality from breast cancer. According to the American Radiological Society, 1 in 6 breast cancers occur in women in their 40s. Studies show that 75% of women diagnosed with breast cancer have no family history and are not considered high risk. Mammography aids in early detection and treatment and improves overall patient outcomes. Since 1990, these screenings have reduced her breast cancer death rate in the United States by nearly 40%. The UPSTF predicted that the change alone “could increase lives saved by 19 percent.”
Second, UPSTF’s work will help address existing health disparities related to breast cancer. According to the ACS report, black women have a 4% lower incidence of breast cancer than white women, but a 40% higher mortality rate from breast cancer. Black women are also more likely to be diagnosed with triple-negative breast cancer, which is aggressive and difficult to treat. In addition, they are more likely to carry dangerous genetic mutations. Studies continue to show that screening may help black women detect breast cancer when treatment is likely to be effective.
The UPSTF’s decision to recommend a starting age of 40 years for breast cancer screening is welcome. However, additional research on health disparities is needed, as indicated by an independent commission.
