New Smartwatch Metrics Refer to Overall Heart Health
Wearable technologies such as smartwatches provide doctors with new ways to monitor and detect patient health Cardiovascular risk factors. Metrics such as daily steps total and average resting heart rate are already used to predict cardiovascular health. However, Zhanlin Chen, a medical student at Northwestern University in Chicago, Illinois, has developed a new combination metric that can provide a more reliable indicator of heart health.

“Daily steps are known predictors of cardiovascular health,” Chen said. “But that's an indirect indicator. [only] It shows how much we move. I hope this will lead to predictions of the outcome. ”
Such indirect data does not provide a big picture. For example, two people may take 10,000 steps per day, but if their hearts have a much higher heart rate, it can be a sign that their heart is working harder than it should be. Similarly, the heart rate outside the context of a person's physiological movement will also feel incomplete. Combining these two metrics could potentially provide deeper insights by dividing the average daily heart rate by the number of steps per day. “When you combine these two measurements (heart rate and step count), this is a better indicator of how the heart is doing than just these two measurements.”

Chen and colleagues used data from 7,000 adults who provided data from Fitbit smartwatches and electronic health records to be used in a national research program, a national prospective study supported by the National Institutes of Health. They found that cardiovascular outcomes were worse than those in the quartile with the lowest heart rate per step (top 25% people).

Eugene Yang, MD, cardiologist at Washington University Seattle University, said he is studying the use of wearables and other new technologies, but said the new metric, which should be validated using finer randomisation or prospective research, is a promising sign of how future wearable technologies can be used to improve people's health.
“We're in a really exciting time,” he said. “If you can accurately measure a lot of things like blood pressure, oxygen saturation, heart rate, daily activity, sleep, and more, it's much stronger in terms of risk prediction.”
Chen hopes that his new metrics will be validated in further research and incorporated into clinical calculations of cardiac risk, and currently does not take into account people's fitness or other environmental and physiological factors.
“We can consider incorporating wearable data into cardiovascular disease risk estimates,” Chen said. “We hope that will lead to changes in medical interventions and risk stratification and screening.”
