Lowering bad cholesterol levels could reduce the risk of dementia by 26%, studies suggest.

People with low levels of low-density lipoprotein cholesterol (LDL-C) in the blood have a lower overall risk of dementia, particularly for Alzheimer's disease. According to published research In the Journal of Neurology Neurosurgery & Psychiatry.

Researchers collected the data 571,000 people in Korea who had not been diagnosed with dementia – 192,213 patients with LDL-C levels below 1.8 mmol/L and LDL-C levels above 3.4 mmol/L (>130 mg/dl).

Analysis of subsequent diagnosis of dementia showed that LDL-C levels with LDL-C levels below 1.8 mmol/L are associated with a 26% reduction in the risk of dementia and a 28% reduction in the risk of Alzheimer's disease, particularly compared to LDL-C levels above 3.4 mmol/L.

Statins appeared to provide additional protection against dementia in the presence of low LDL-C levels. Among those with LDL-C levels below 1.8 mmol/L, statin use reduced the risk of dementia by 13% and reduced the risk of Alzheimer's disease by 12% compared to non-users.

This is an observational study and it cannot draw solid conclusions about causes and effects. The authors also acknowledged several limitations, including focusing on baseline LDL-C levels where lipid profiles can change over time.

Nevertheless, they concluded: “Low LDL-C levels… lower the risk of dementia, including Alzheimer's disease-related dementia, and statin therapy has additional protective benefits.”

Dr. Francesco Tamanini, a neurophysiologist at the University of Reading, said he was not involved in the study.

“This paper examines the correlation and potential causal relationships between high levels of bad cholesterol and risk of dementia. The results provide a compelling argument for researchers to consider LDL cholesterol in addition to classical approaches.”

“However, the risk of dementia is complex and influenced by many factors. Without a detailed understanding of what is happening in the brain, we don't know if there is a direct link between lower cholesterol and lower risk of dementia.
