Study explores acceptability of teleneurology across neurological disorders
One of the first studies to examine patient acceptability of teleneurology and factors influencing acceptability across the full range of neurological conditions found that teleneurology was highly acceptable to a wide range of patients with a variety of neurological diagnoses, including headaches, movement disorders, and other neurological conditions and diagnoses. The study also found that increasing medical complexity (having additional conditions) was associated with increased patient satisfaction with teleneurology.
Elderly patients were as accepting of teleneurology as younger patients, who are thought to be more comfortable with technology. Living in a rural area, where face-to-face consultations are less convenient, was not associated with greater acceptance of teleneurology.
“There will always be times when a specific clinical condition or patient preference dictates that an in-person consultation is essential, but for most patients, the majority of what we're trying to do — understanding their symptoms, when they started, what helps them and what makes them worse — all of the fundamental things we learn in medical school that we know are at least 75 percent of getting a correct diagnosis — can be done over video. Then we can tailor most parts of the physical exam to be done over video as well,” said Linda S. Williams, MD, PhD, a research scientist at the U.S. Department of Veterans Affairs and Regenstrief Institute and lead author of the study who has a clinical neurology practice specializing in stroke patients. “This is true regardless of the health care system.”
“Telemedicine is more efficient. It may help us make better use of scarce resources at a time when there is a physician shortage, and most importantly, it may help us meet patients at home when they are struggling with a condition such as epilepsy, which is very important in neurology, or when they are unable to drive.”
Patients who participated in the study were surveyed 2 weeks after visiting a clinician through teleneurology. The visit took place either at home or via video to an outpatient primary care clinic with telehealth capabilities.
“Older patients and those with more complex medical problems often make up a larger proportion of those who need to be cared for by a neurologist,” said lead author Courtney R. Siegel, PhD, a medical student at the Indiana University School of Medicine. “Our findings should help health care providers know that they should not preclude considering teleneurology for these patients.”
“There is a significant shortage of neurologists in the United States, and neurologists, like many other medical specialties, tend to be more dense in urban areas and less dense in rural areas,” Dr. Williams said. “Teleneurology is one way to deliver care more efficiently. Telehealth allows us to see more patients in a day than if we had to drive to a single remote clinic, spreading out scarce resources.”
